Prize Winnings Report Form

As a condition of entry, a bowler who has won a total of $300 or more in any event, $500 in all available events of any tournament or $1000 or more in the last 12 month period - must, at the time of submitting the entry blank, report their actual score (s), position (s) and amount won. (USBC Rule 319D)
